• Made application form( Ro 1/1) with all document at Phuket Animals Quarantine station at least 7 working days before departure
• The animals must be inspect for health condition by Veterinary officer at Phuket animal Quarantine station at least 3 working day before departure
• The animals should be not less than 4 month old
• Export licence and Health Certificate valid in 10 days

Document Required

• Passport of the owner (original & copy)
• Rabies Vaccination certificate ( original & copy)
• Last Rabies Vaccinated should be 1 month before departure
• Another Vaccine Required by destination country
• Microchip or Tattoo certificate
• Import permit form destination country
